Telehealth Consent

By using ReBrandMD's platform and submitting a health assessment, you understand and agree that certain services may be provided through telehealth by independent licensed medical providers.

Nature of Telehealth

Telehealth allows healthcare providers to evaluate, diagnose, consult, and provide certain treatment recommendations remotely using electronic communications and health information submitted through online forms, messaging, video, audio, or other digital tools.

Telehealth is not appropriate for every medical condition. In some cases, a provider may determine that telehealth is not sufficient and may recommend in-person care, additional evaluation, laboratory testing, or referral to another healthcare professional.

Independent Medical Providers

Medical services are provided by independent licensed medical providers through the Asher Med provider network or other affiliated provider networks. ReBrandMD does not practice medicine and does not employ or control the medical judgment of these providers.

Your provider will review your health assessment, medical history, treatment goals, and any other relevant information you submit. The provider may approve treatment, deny treatment, request additional information, or recommend a different course of care.

Prescription Decisions

Prescription decisions are made solely by the licensed medical provider. ReBrandMD does not prescribe medications and does not determine whether a medication is appropriate for you.

If a provider determines that treatment is medically appropriate, a prescription may be issued and sent to a licensed pharmacy partner for fulfillment.

Risks and Limitations

Telehealth has benefits, including convenience and access, but it also has limitations. These may include incomplete information, technology issues, delays in communication, or the inability to perform an in-person physical examination.

You agree to provide accurate, complete, and truthful information. Failure to provide accurate information may affect provider review, treatment eligibility, safety, and outcomes.

Emergency Care

Telehealth services offered through this platform are not for emergencies. If you are experiencing a medical emergency, call 911 or seek immediate emergency care.
